Fee and Insurance Information

 

  
Fees determined according to surveys of usual customary charges in this area. Assignment accepted for services covered by Medicare (which pays 80% of the fee if deductible met) and Medicaid (pays the charge in full). Deductible and copay amounts (20% or part not covered by supplement are payable at time of service. NOTE: Medicare does not cover refraction services (determining the prescription for glasses), Medicaid sometimes does, but usually only every one to two years and not all Medicaid beneficiaries are eligible.

Payment at time of service on private insurance and cash accounts. We will submit insurance claim or furnish forms upon request.

Managed care (HMO's, PPO's) and Vision Plans: changing very rapidly. Please Inquire
